Services: Counseling and clinical supervision are professional services provided by Anchor of Hope Counseling & Clinical Supervision, PLLC. These services are not a substitute for medical, emergency, or legal care.
No Guarantee: I understand that outcomes cannot be guaranteed, and progress depends on my participation and circumstances.
Confidentiality: I understand that my information is confidential, except when disclosure is required by law (e.g., risk of harm to self or others, abuse of minors or vulnerable adults, or court orders).
Telehealth: If services are provided via telehealth, I consent to remote sessions and acknowledge potential limitations of electronic communication.
Payment: I am responsible for fees, co-pays, or self-pay arrangements. Insurance coverage is my responsibility.
Limitation of Liability: I release Anchor of Hope Counseling & Clinical Supervision, PLLC from liability for any indirect or consequential outcomes related to services or website use.
Governing Law: These terms are governed by the laws of the State of Michigan.

Confidentiality: My information is protected under federal and Michigan law. Personal information shared during counseling or supervision will remain confidential except when disclosure is required by law (e.g., risk of harm, abuse, or court orders).
Use of Information: My information may be used to provide services, schedule appointments, process payments, and maintain records.
Electronic Communication: I understand that email, online forms, and telehealth platforms may have inherent privacy risks.
Data Security: The practice takes reasonable steps to protect my information, but no system is completely secure.
Third-Party Sites: The practice is not responsible for the privacy practices of external websites linked from its site.

Anchor of Hope Counseling & Clinical Supervision, PLLC – Refund Policy

Session Fees: Payment is due at the time of service. Completed counseling or supervision sessions are non-refundable.
Cancellations: I understand that at least 24 hours’ notice is required to cancel or reschedule an appointment. Late cancellations or missed appointments may be charged the full session fee and are not billable to insurance.
Prepaid Sessions: Refunds for unused prepaid sessions may be issued upon request, minus any administrative fees. Completed sessions are not refundable.
Billing Concerns: I agree to contact the practice within 14 days if I believe there is a billing error.
